Maintenance Record Template for Support Requests

Use this maintenance record format to document inspections, service activities, alarms, part issues, and support evidence for vertical injection molding machines.

Maintenance records make support work more direct. Without a record, a team may need to reconstruct days or weeks of machine history from memory. With a record, support can see what was inspected, what was changed, and what happened afterward.

Use the template below as a starting point and adapt it to your factory procedure.

Basic Machine Information

  • Machine model:
  • Machine number or factory asset ID:
  • Production line:
  • Mold or product:
  • Material:
  • Program name:
  • Date and shift:
  • Person recording:

Inspection or Service Activity

  • Activity type: daily inspection, scheduled maintenance, repair, mold change support, alarm review, or other.
  • Area checked: lubrication, hydraulic, heater, sensor, cable, table, clamp, injection, safety device, auxiliary signal, or other.
  • Visible condition:
  • Action taken:
  • Person responsible:
  • Machine stopped or running:
  • Photos attached:

Production Condition After Activity

  • First-cycle observation result:
  • Any alarm:
  • Any abnormal sound or movement:
  • Any part defect:
  • Any new parameter change:
  • Whether production continued:

If a problem appears after maintenance, record the exact point where it appears. “After maintenance the machine stops” is less useful than “after maintenance, the machine stops during table movement before mold close.”

Evidence to Attach

Attach controller screenshots, safe machine photos, short safe video, and related parameter change records. If the issue involves auxiliary equipment, include its screen photo and program name if available.

FAQ

Support review questions

Why keep a maintenance record if support can inspect the machine later?
A record shows what changed before the issue appeared. It helps support separate routine wear, recent service activity, production setup, and operator workflow factors.

Need help reviewing this machine condition?

Send the machine model, controller screenshots, current mold or product, recent changes, and safe photos or video. Deboge can review the information and suggest the next support step.
